Description: This session will explore the essential roles of multidisciplinary team members and effective approaches to retain patients on home dialysis, illustrated through case studies. 

Presenter: Preethi Yerram MD 

Bio: Dr. Yerram is a Professor of Medicine in the Division of Nephrology at the University of Missouri - Columbia, and a Staff Nephrologist at the Truman VA Hospital. She is also the Medical Director for the University of Missouri Health Care's Kidney Transplant Program. She served as the medical director for home/transitional care units as well as acute dialysis units in Columbia, MO and currently serves as the President-Elect of the ‘National Forum of ESRD Networks’ and Chair of the Forum’s Medical Advisory Council. She is also the lead facilitator for the Missouri Telehealth Network's ‘Kidney Disease ECHO'. In her multiple roles, Dr. Yerram's focus is on improving the care of patients with kidney disease.
